Senior & Teen Year Magazine Model Release

To submit images to Senior Year Magazine, for either their Senior or Teen magazine, you will need an approved Model Release.   A Template for the approved version of the model release form is available for the Advanced Builder so you can create your own App to send to your clients and have on your phone ready for when you need it. 

This model release is not intended to take the place of any model release you already use to cover the use of images for your own business use and should be completed in addition so that you have it when you later decide to submit images for inclusion in either of the Senior Year magazines.

TIP:  If you need a MyPhotoApp account, just go to http://MyPhotoApp.com where you can sign up for either a Standard or Advanced account.
 
To create your copy of the Senior Year Magazine Model Release for your personal use follow these steps:

In the Advanced builder click on the TEMPLATES TAB to view the available templates. If you can't see the Senior Year magazine model Release template at the top  of the list just type in Senior Year Magazine in the search box and click the magnifying glass to show it like this:



Click the "Create New App using this Template" Rocket button at the bottom to open the create app dialog in the builder. The templates details will already be in the shared template field,  enter the name you want to call the app and then click the Create App button at the bottom.   

You now have your own copy of the model release app. 

Customize the Model Release App

Before you use your app you will need to customize it to add your business details by following these steps: 

You will see in the model release this sentence

I agree that YOUR BUSINESS NAME has my permission to submit 

You need to change the words  YOUR BUSINESS NAME  to the name of your business.  
To do this either click on 
  • the setting cog next to the Form in the layout Setting column  or
  • Anywhere on the Form text in the Visual Design to open the model release form to edit it.
The Top Field on the left is the one that contains the text you need to change so click on this to select it. 
You can now delete the words  YOUR BUSINESS NAME and type in your business name instead. 
Click Save to update the form with the changes.



Before you can use your app you need to publish it by clicking on the Publish App button.   This will open the Publish Status dialog


You can now text the App URL to your phone from the Publish Status dialog and save it to your home screen for future use. 

How To Use The Model Release

You can get your client to fill out and sign the model release in one of two ways:

  1. You can open the Model Release on your phone and hand it to them to fill it out or
  2. You can open the Model Release on your phone and instantly share it with them as a text or email by pressing one of the share buttons in the app.  They will then receive a copy of the release and can fill it out on their device. 

Once they have filled out the release and signed it, all they need to do is press the Submit button to send the Model Release to you.   Each time the form is filled in and submitted it is cleared ready for the next time you need to use it. 

How will  I receive a copy of the Model Release?

When the release is submitted you will automatically receive an email to your registered email address. The form submission is also stored on the MyPhotoApp server and retained for 6 months. You can access your form submissions by clicking on the Forms Data tab in the builder.    

Here you can:
  • Filter form submissions by the app name or other detail
  • Download a CSV file of all form submissions
  • View forms in a new browser tab by clicking the form icon on the left  this allows you to Print or Save your form as a PDF using the print options from your browser. 
  • Delete forms by clicking the trash can next to the form submission entry

Will the client also receive a copy of the model release?

The client will NOT receive a copy by default but there are 2 ways to provide a copy of the model release to your client:
  1. You can forward a copy of the email you receive with the form submission to your client or
  2. You can edit the model release form and change the simple text line field for their email to an email field and make sure that the box send a copy of this submission to this email is checked and republish the app.   Now when ever someone fills in and submits the model release they will then also automatically get a copy of it.
