Info Request Shared Template

The new template bazaar  on the template tab in the builder is where you will find templates that members  have shared. 

But here is an example of a simple info request template shared by Nick Porcaro together with the shared template info to use to create your own version of it.

What Nick had to say about his template  " I set this InfoRequest App up as a shared template that anyone with MPA can use. Just make sure you modify things like the logo and where the info requests get mailed to.  I've got a few different online info request forms I use for different types of photography and decided to consolidate them into one using MPA and its pages feature. I'm usually pretty it up some with photos but thought I'd share this  template with the other MPA users. I'm finding tons of ways to use MPA that I hadn't even considered when I first bought it"
